You are welcome to attend at either Brent House in Bridgwater or at our branch surgery in Westonzoyland. Full details of how to register are available at either surgery. Please contact reception where you will be asked to fill in a registration form or bring in your NHS medical card.

We would like to obtain your medical history as soon as possible so you will be asked to complete a medical questionnaire. This will enable us to provide the best possible medical care for you and your family.

PreRegistration If you wish to register, please call in to the surgery to collect a registration pack or, if you prefer, complete the forms below, print at home and drop them into the surgery. Please note that completing these forms does not guarantee that you will be accepted as a patient, please talk to the Receptionist when you attend.
